Victim Input Form for First Appearance

This victim input form is a way for the victim of a crime to notify the State Attorney’s Office of his or her views regarding the defendant’s release to be shared with the Judge at the defendant’s first appearance hearing.

Requests for the charges to be dropped will NOT be considered through use of this form. Any such requests must be made in person, directly to the Assistant State Attorney or Victim Witness Specialist assigned to the case. Please call the State Attorney’s Office Victim Witness Department at (727) 464-6090 to schedule that appointment.
